AmeriHealth Caritas Next, our individual and family health plans both on and off the Health Insurance Marketplace, is available for the residents of Louisiana in the listed parishes. Participating on the Health Insurance Marketplace aligns with our vision to empower those in need across their full life journey by providing a health insurance option for those who otherwise would have no access to health insurance coverage through their employer, Medicaid, or Medicare. We aim to continue to be there for our members as they transition out of Medicaid and can gain coverage through the Health Insurance Marketplace.

We are currently developing a network of hospital, physical, ancillary, and behavioral health care providers in the listed parishes, and we want to partner with dedicated providers like you.

To request a Provider Agreement, please complete a Provider Contract Inquiry Form (PDF) and return by email to network@amerihealthcaritasla.com.

Council for Affordable Quality Healthcare (CAQH) credentialing

Our goal is to make the credentialing application process as easy as possible, and we encourage all providers to utilize the Council for Affordable Quality Healthcare (CAQH) system. Please provide us with your CAQH ID number and grant us access to your information.

If you are not currently a CAQH-participating provider, we highly recommend subscribing at caqh.org. If you do not wish to become a CAQH-participating provider or if you are an organizational provider applying for participation with AmeriHealth Caritas Next, please call Provider Services at 1-833-315-2252.
