Transition of Care Program

The Transition of Care program provides a safe transition from hospital to community or nursing home to community. Discharge planning begins at time of admission.

We reach out to members to:

• Ensure their discharge needs are met.
• Ensure they understand discharge instructions.
• Provide medication education.
• Assist with scheduling discharge follow-up and provider appointments.

The member is offered assistance in scheduling provider follow-up. The goal of the proactive outreach is to ensure the member is safely discharged and prevent readmission.
